Professional medical care and rehabilitation
A Sub-acute medical care and rehabilitation unit’s purpose is to bridge the gap where a patient is medically stable to go home after an operation or illness. The patient would still be dependent on 24 hour professional medical care and further rehabilitation. No technological advanced equipment or monitoring are required, but medical care is needed.
A multidisciplinary team meets regularly to discuss patient progress and to plan their rehabilitation for recovery.
